9/11 - who got blamed for making any mistakes?

Just a quick question: has Bush held anybody, even just one person, responsible for making a mistake, or any mistakes that allowed 19 terrorists to take flying lessons, and then hijack four Airliners that then led to the Murder of 3000 Americans on 9/11?

I ask because maybe he has and just isn't letting on and some commentators do accuse the Bush aministration of being more secretive than even the Nixon administration.
